It is when someone is the third person with the other two having romantic relationship and would prefer to be left alone by this third guy. In this sense, the third guy is said to be playing gooseberry, while accompanying the two.
US English:
The term third wheel refers to someone who deters the socialization of a couple, perhaps when being invited out of pity or through a feeling of duty.
Generally, a dirty onlooker, at someone's wife, considers the husband - kabab me haddi.
